Library staff can access the online Ontario Health system for you, and print a proof of vaccination for a small fee.

You need to provide:

  • your green photo health (OHIP) card (numbers from both the front and back of the card, expired cards will be accepted)
  • your date of birth (also shown on the health card)
  • your postal code (the one associated with your health card)
  • consent to access your online vaccination record

Please note: we print but do not laminate the official document. You can choose paper (25c), or we can shrink it and print on waterproof NeverTear synthetic ($1.00).

If you do not want to provide your health card to Library staff, we can instead help you to print a copy yourself on the library computers or help you to use our remote printing service.

If you have a red and white health card, call the Provincial Vaccine Booking Line at 1-833-943-3900. The call centre agent can email you a copy of your proof of vaccination.
